1. General Information

This form has been prepared pursuant to the Turkish Personal Data Protection Law No. 6698 (“KVKK”) so that data subjects may exercise the rights set out in Article 11 of the KVKK and submit their applications to ÇAKMAKÇI KIYMETLİ MADENLER SAN. TİC. A.Ş. in its capacity as data controller. Your requests may be submitted by completing this form and using one of the methods below:
